Back pain killing your sex life? Study reveals best positions to save your spine

Contrary to popular belief, spooning is not always the best sex position for those with a bad back, according to new research from the University of Waterloo. For the first time ever, scientists have successfully documented the way the spine moves during sex and discovered exactly why certain positions are better than others when it comes to avoiding back pain. The pioneering study combined infrared and electromagnetic motion capture systems to track how ten couples' spines moved when attempting five common sex positions. The findings were used to create an atlas, or set of guidelines, that recommends different sex positions and thrusting techniques based on what movements trigger a patient's pain. Contrary to popular belief, spooning is not always the best sex position for those with a bad back, according to new research from the University of Waterloo.

For the first time ever, scientists have successfully documented the way the spine moves during sex and discovered exactly why certain positions are better than others when it comes to avoiding back pain.

"Any family doctor will tell you that couples often ask them how to manage their back pain during and after sex. Many couples will remain celibate because one night of love-making can lead to months of back agony," said Professor Stuart McGill, of Waterloo's Faculty of Applied Health Sciences. "Until now, doctors have never had any hard science to base their recommendations upon."

The pioneering study combined infrared and electromagnetic motion capture systems -- like those used in the creation of video games -- to track how 10 couples' spines moved when attempting five common sex positions. The findings were used to create an atlas, or set of guidelines, that recommends different sex positions and thrusting techniques based on what movements trigger a patient's pain.

"Before now, spooning was often recommended by physicians as the one position that fit all. But as we've discovered, that is not the case," said Natalie Sidorkewicz, a PhD candidate at Waterloo and lead author on the paper. "Sex positions that are suitable for one type of back pain aren't appropriate for another kind of pain."

The atlas recommends that men who are flexion-intolerant -- meaning those whose back pain is made worse by touching their toes or sitting for long periods of time, for example -- replace spooning with doggy-style sex. The guide recommends that these men use a hip-hinging motion rather than thrusting with their spines.

"For the first time ever, we now have very solid science to guide clinicians on their recommendations for patients who suffer debilitating back pain, but still want to be intimate," said Sidorkewicz. "This has the potential to improve quality of life -- and love-life -- for many couples."

According to Statistics Canada, four of every five people will experience at least one episode of disabling low back pain in their lifetime. Up to 84 per cent of men with low back pain and 73 per cent of women report a significant decrease in the frequency of intercourse when suffering back pain.

The study also shed light for the first time on the mechanics of the male orgasm. Electrodes hooked up to the male participants' muscles revealed that it is abdominal and buttock, not back muscles, that are most active during orgasm. Spine motion, on the other hand, varied with the individual. For some males, a drastic increase in flexion or extension was seen, while for others spine motion did not change much at all.

"Many of the back pain patients that we see have told us that they experience elevated levels of pain during orgasm, to the point where they will avoid having one during sex with their partner," said Sidorkewicz. "These initial findings help us to begin to understand what might be provoking their pain during the moment of climax."

A paper documenting male spine movement was published in the journal Spine. Female findings are expected to be published in the coming months. The next phase of the study will involve recruiting patients with hip pain and additional categories of back pain to further develop the guidelines.

Artificial light, biological clock disruptions, increase breast cancer risk, study finds

The disruption of a person's circadian rhythm -- their 24-hour biological clock -- has been linked to an increased risk of breast cancer, according to new University of Georgia research. The culprit, in this study in particular, is artificial light.

"Exposure to artificial light leads to a significantly higher risk for developing breast cancer," said Chunla He, a biostatistics graduate student in the UGA College of Public Health. "To decrease the use of artificial light, people should avoid working at night and implement earlier bed times."

Her research, published in the International Archives of Occupational and Environmental Health, examined key studies that included risk factors for developing breast cancer.

"A large body of related research about circadian rhythms and breast cancer exists," He said. 
"However, these studies are inconsistent and have a variety of limitations."

Under the mentorship of Sara Wagner Robb, assistant professor of epidemiology in the College of Public Health, He turned to previous studies to see what the research revealed.

In addition to other relevant exposures, He examined studies on breast cancer and flight attendants, who typically work both day and night shifts. The flight attendants represented a group of workers particularly susceptible to disrupted circadian rhythms, which are heavily influenced by light.

In her analysis, He found that employment as a flight attendant was related to an increased risk of breast cancer.

"People naturally secrete the hormone melatonin, which helps to regulate the circadian rhythm," He said. "When the sleep-wake cycle is disrupted by artificial light, melatonin secretion is adversely affected."

Robb recommends spreading this information to shift workers so they understand the harms in disrupting their circadian rhythms.

"Individuals engaging in this type of work should be aware of these risks and may make efforts to adjust their circadian rhythms," she said. "Although additional studies are certainly needed, scientists are becoming increasingly aware of the health risks associated with night workers and others exposed to circadian-disrupting behaviors."

Robb and He also advise that future research needs to examine social constraints -- which may foster disruption of circadian rhythms -- on shift workers. Additionally, shift workers should contact their primary care physicians for personalized treatment and options.

"This information tells us the harm in disrupting our natural cycle," He said. "With this new analysis, we must be cautious in our exposure to artificial light."

Helping outdoor workers reduce skin cancer risk

Skin cancer is one of the biggest fears for one in two outdoor workers and when the boss and staff work together the sun safe message gets through, a QUT study has found.

The study, which found more than 50 per cent of outdoor workers rated UV radiation exposure at work as one of their biggest concerns, also identified how a workplace intervention could improve workers' behaviours and attitudes towards sun protection to reduce their risk of skin cancer.

QUT in collaboration with Cancer Council Queensland and Curtin University worked with 14 Queensland outdoor workplaces from farming, construction, public service and local government industries to develop personalized sun protection action plans.
After adopting these individualized plans, the number of workers who reported using sun protection increased significantly.

Professor Michael Kimlin and Associate Professor Monika Janda, from QUT's AusSun Research Lab, led the intervention program and the results have been published in the Journal of Occupational and Environmental Medicine titled "Changes in Outdoor Workers' Sun-Related Attitudes, Beliefs and Behaviours: A Pre-Post Workplace Intervention."

"The message is simple, outdoor workers who see their workplace, supervisors and bosses supporting sun protection measures will follow their lead," Professor Janda said.

"Providing better programs, which take into account the specific workplace tasks and culture can make a difference."

QUT health promotion specialist Dr Marguerite Sendall, who was involved at the grassroots level in implementing the workplace interventions, said the study worked closely with workplaces to develop and implement a tailored sun safety plan, encouraging sun awareness and supporting sun protection practices at work.

"The program was about working together in partnership with workplaces, taking into account their individual circumstances and environment, and developing realistic and sustainable strategies," Dr Sendall said.

"It was this partnership and customized strategies that made this study a success and led to significant improvements in the way workplaces and employees approached sun safety."

Dr Sendall said after the 12 months intervention, the results revealed when a workplace was sun safe there was a significant improvement in the attitudes and behaviours of outdoor workers when it came to sun protection.

"For example, the proportion of workers who checked their skin for early signs of skin cancer increased after the intervention program, with 80 per cent reporting they had conducted a skin check in the previous 12 months," Dr Sendall said.

"Ten per cent more workers also had their skin checked by a doctor.
"The study found after the intervention, 20 per cent more workers said they usually or 
always seek natural shade, 25 per cent more workers wore broad-brimmed hats, 19 per cent more wore long-sleeved collared shirts and 16 per cent wore long trousers," she said.
Dr Sendall said there was also a shift in outdoor worker attitudes.

"The proportion of workers who agreed their workplace enforced sun protection and agreed their supervisors protected themselves increased by 10 per cent to 76 per cent of all workers," she said.

One of the workplaces to take part in the study was Goondiwindi Regional Council and workplace champion Andrew Singh said the tailored intervention had produced tangible results.

"At our council, after introducing the intervention, the road construction crew adopted a number of sun safe initiatives," Mr Singh said.

"For example, the council provided portable shade structures to be used during breaks, vehicle windows were tinted and our staff swapped baseball-style caps for broad-brimmed hats.

"Overall, what we found was workers were keen to follow these sun safe measures as they saw their initiatives were welcomed and keenly supported by the mayor, councillors and management."

Dr Sendall said the study highlighted the importance of a consistent sun safe workplace culture.

"Despite ongoing public health campaigns, outdoor workers remain a difficult to reach group but if we can take care in making the sun protection program really relevant to their personal circumstances and work environment, the potential health benefits are significant."

Switching to vehicles powered by electricity from renewables could save lives

Driving vehicles that use electricity from renewable energy instead of gasoline could reduce the resulting deaths due to air pollution by 70 percent. This finding comes from a new life cycle analysis of conventional and alternative vehicles and their air pollution-related public health impacts, published Monday, Dec. 15, 2014, in the Proceedings of the National Academy of Sciences.

The study also shows that switching to vehicles powered by electricity made using natural gas yields large health benefits. Conversely, vehicles running on corn ethanol or vehicles powered by coal-based or "grid average" electricity are worse for health; switching from gasoline to those fuels would increase the number of resulting deaths due to air pollution by 80 percent or more.

"These findings demonstrate the importance of clean electricity, such as from natural gas or renewables, in substantially reducing the negative health impacts of transportation," said Chris Tessum, co-author on the study and a researcher in the Department of Civil, Environmental, and Geo- Engineering in the University of Minnesota's College of Science and Engineering.

The University of Minnesota team estimated how concentrations of two important pollutants -- particulate matter and ground-level ozone -- change as a result of using various options for powering vehicles. Air pollution is the largest environmental health hazard in the U.S., in total killing more than 100,000 people per year. Air pollution increases rates of heart attack, stroke, and respiratory disease.

The authors looked at liquid biofuels, diesel, compressed natural gas, and electricity from a range of conventional and renewable sources. Their analysis included not only the pollution from vehicles, but also emissions generated during production of the fuels or electricity that power them. With ethanol, for example, air pollution is released from tractors on farms, from soils after fertilizers are applied, and to supply the energy for fermenting and distilling corn into ethanol.

"Our work highlights the importance of looking at the full life cycle of energy production and use, not just at what comes out of tailpipes," said Bioproducts and Biosystems Engineering Assistant Professor Jason Hill, co-author of the study. "We greatly underestimate transportation's impacts on air quality if we ignore the upstream emissions from producing fuels or electricity."

The researchers also point out that whereas recent studies on life cycle environmental impacts of transportation have focused mainly on greenhouse gas emissions, it is also important to consider air pollution and health. Their study provides a unique look at where life cycle emissions occur, how they move in the environment, and where people breathe that pollution. Their results provide unprecedented detail on the air quality-related health impacts of transportation fuel production and use.

"Air pollution has enormous health impacts, including increasing death rates across the U.S.," said Civil, Environmental and Geo- Engineering Associate Professor Julian Marshall, co-author on this study. "This study provides valuable new information on how some transportation options would improve or worsen those health impacts."

Software to automatically outline bones in x-rays

Research into disorders such as arthritis is to be helped by new software developed at the University of Manchester which automatically outlines bones -- saving thousands of hours of manual work.

Amidst a national shortage of radiographers in the UK and an increasing requirement for researchers to work with large databases of radiograph images, the software which is being funded by the Engineering and Physical Sciences Research Council, is being designed to automatically pick out the shapes of bones in the images, rather than relying on individual researchers.

The system can already identify hips, but the researchers from the University's Institute of Population Health will now adapt it to map out knees and hands and to be able to learn to identify other bones and structures within the body.

The funding will allow further development to ensure the system is accurate enough that it can be used in hospitals to help provide faster diagnosis of problems in patients.

Professor of Computer Vision, Tim Cootes said: "Mapping the outlines of bones from radiographs is hard work that takes time and skill. When researchers into conditions like arthritis are working with hundreds of images, it's a very inefficient way of obtaining data.

"The idea of this software is to take the routine tasks out of human hands, so scientists can 
focus on drawing conclusions and developing treatments."

The funding of £300,000 lasts for three years and builds on earlier work which developed software, called Bonefinder, to identify problems and find the outlines of hips. This free software has been adopted by a number of research groups, including some based in Oxford and California.

Professor Cootes added: "We have a growing problem with arthritis which affects more than 30% of over 65s and costs around £30 billion to the UK economy year.

"Ultimately we want to get this technology into hospitals where it can save time and resources for the benefit of patients."

Researchers from the UPM have characterized the health risks derived from the usage of mercury in artisanal gold mining in Colombia through probabilistic models.

The tools developed by researchers from the Environmental Geochemistry Research and Engineering Laboratory (LI2GA) of the Universidad Politécnica de Madrid (UPM) have given evidences of their quantitative efficiency to problems about occupational and environmental exposures to pollutants. These tools will allow researchers to categorize risk situations and to give priority to the intervention performances, especially in regions with limited financial resources where is more complex to conduct rigorous clinical studies.

Artisanal gold mining is common in Latin-American regions, Africa and Southeast Asia where there are large socio-economic inequalities and active or abandoned goldfields. This activity is performed outside of the workplace health and safety regulations. Therefore, miners can be at risk due to possible accidents during the exploitation of deposits and due to chronic exposure to mercury. This risk of mercury poisoning is not just limited to workers but also the entire population of the mining communities.

Mercury is a toxic element that can cause neurological disorders in humans or other organism. Common exposures include inhalation of elemental mercury or consumption of fish contaminated with mercury. Mercury from artisanal gold mining is used to concentrate gold in an amalgam of both elements. Later, it is burned in order to rid of the mercury and to recover the gold. During this amalgamation process, accidental and intentional discharges of mercury can occur.

Mercury eventually ends up in nearby rivers and, consequently, in the fish tissues. The problem is that fish constitutes an essential part of the diet for the mining communities included in this research. Likewise, amalgam burning is made in small workshops, sometimes in houses. All this can cause mercury pollution in the air of workplaces, houses and outer areas of these communities.

Researchers from LI2GA of UPM collected data about biometrics, lifestyle and consumption of 12 gold mining departments in Colombia. They also collected mercury concentration air data of in the amalgamation workshops, air data of the roads of these mining communities and also data of the diverse fish species of the area.

All this data along with the quantitative data of potential toxicity of mercury were conducted by probabilistic risk assessment. The results unveiled the mercury exposure dose in these mining communities exceeded the admissible values, even reaching 200 times higher of the reference values in certain maximum exposure situations.

The general population is also exposed to unacceptable risk to health (up to 50 times higher than the recommended values) due to consumption of contaminated fish and breathing mercury vapor in the air.

The probabilistic risk assessment cannot replace the clinical research to control public health and intervention. However, it can be a useful tool to characterize and to highlight these problems in order to give priority to corrective performances.

Environmental costs, health risks, and benefits of fracking examined

A strange thing happened on the way to dealing with climate change: Advances in hydraulic fracturing put trillions of dollars' worth of previously unreachable oil and natural gas within humanity's grasp.

The environmental costs -- and benefits -- from "fracking," which requires blasting huge amounts of water, sand and chemicals deep into underground rock formations, are the subject of new research that synthesizes 165 academic studies and government databases. The survey covers not only greenhouse gas impacts but also fracking's influence on local air pollution, earthquakes and, especially, supplies of clean water.

The authors are seven environmental scientists who underscore the real consequences of policy decisions on people who live near the wells, as well as some important remaining questions.
"Society is certain to extract more gas and oil due to fracking," said Stanford environmental scientist Robert Jackson, who led the new study. "The key is to reduce the environmental costs as much as possible, while making the most of the environmental benefits."

Fracking's consumption of water is rising quickly at a time when much of the United States is suffering from drought, but extracting natural gas with hydraulic fracturing and horizontal drilling compares well with conventional energy sources, the study finds. Fracking requires more water than conventional gas drilling; but when natural gas is used in place of coal or nuclear fuel to generate electricity, it saves water. From mining to generation, coal power consumes more than twice the water per megawatt-hour generated than unconventional gas does.

Unconventional drilling's water demand can be better or worse than alternative energy sources, the study finds. Photovoltaic solar and wind power use almost no water and emit no greenhouse gas, but cheap, abundant natural gas may limit their deployment as new sources of electricity. On the other hand, fracked gas requires less than a hundredth the water of corn ethanol per unit of energy.

Fracking's impact on both climate change and local air pollution is similar to its impact on water, finds the study "The Environmental Costs and Benefits of Fracking," published in the Annual Review of Environment and Resources.

Getting a fractured well going is more intense than for conventional oil and gas drilling, with potential health threats arising from increases in volatile organic compounds and air toxics.

But when natural gas replaces coal as a fuel for generating electricity, the benefits to air quality include lower carbon dioxide emissions than coal and almost none of the mercury, sulfur dioxide or ash.

Globally, though, relief to climate change is uncertain, the study finds. "While the increased gas supply reduces air pollution in U.S. cities downwind from coal-fired power plants, we still don't know whether methane losses from well pads and pipelines outweigh the lower carbon dioxide emissions," said Jackson.

In the eastern United States, fears of contaminated drinking water have raised more concerns than fracking's water consumption. Gas and chemicals from humanmade fractures thousands of meters underground very rarely seep upward to drinking-water aquifers, the study says. The real threats are failures in the steel and cement casings of wells nearer to the surface and the disposal of wastewater, the study finds. Numerous previous studies have shown that casings fail between 1 percent and 10 percent of the time, depending on geology and well construction.

Cases of groundwater contamination have been hotly debated, but the new study finds that the overwhelming evidence suggests it has happened, albeit not commonly. Is the methane contamination observed in drinking water a precursor to other toxins -- arsenic, various salts, radioactive radium and other metals -- making their way up slowly? The researchers do not yet know. A few recent studies suggest the answer could be "yes" in rare cases.

How oil and gas companies handle wastewater -- fluid used to fracture the shale that flows back up the well and water unleashed with the oil and gas -- shows the importance of state policies. "Wastewater disposal is one of the biggest issues associated with fracking," said co-author Avner Vengosh, a professor of geochemistry at Duke University.

Most fracking wastewater in the United States is injected deep underground, and an increasing amount is recycled for subsequent drilling or sent to advanced water treatment facilities. However, a handful of states still allow the wastewater to be used for watering cattle, sprayed onto roads for dust control or sent to municipal water-treatment plants not equipped to handle the chemicals involved.

All bad ideas, according to the authors of the new survey, who work at Duke University, MIT, Ohio State University, Newcastle University, Los Alamos National Laboratory, the National Oceanic and Atmospheric Administration and Stanford. One study they cite found that the agricultural use of fracking wastewater killed more than half of nearby trees within two years.

Injection of wastewater deep underground presents its own problems, the study finds. The practice occasionally has caused earthquakes strong enough to be felt by human beings, while the fracturing of shale miles below the surface rarely has done so. The dangers of seismicity can be reduced, however, if energy companies follow basic guidelines and undertake careful monitoring.

The study highlights several policies and practices that could optimize fracking's environmental cost-benefit balance, and it highlights the need for further research. For example, the direct impact on the health of nearby residents is virtually unknown. "Almost no comprehensive research has been done on health effects," said Jackson, "but decisions about drilling -- both approvals and bans on fracking -are made all the time based on assumptions about health risks."
